Innovations and celebrations at Medway Maritime Hospital

Dr Sanjay Suman, Consultant Geriatrician at Medway Maritime Hospital, received the award in London on Wednesday 1 March in recognition of his inspiring Proactive Assessment Clinic for the Elderly (PACE). PACE is a community based initiative that supports the health needs of patients over 65 years old to ensure they receive community care close to their homes.

Dr Sanjay Suman said: “It was an honour to receive this award yesterday and I would like to take this opportunity to acknowledge the hard work and dedication put in by the whole team at Medway NHS Foundation Trust. Of course none of this would have been possible without managerial input from Medway Maritime Hospital and the fantastic I.T team at Medway CCG for their collaborative success in accessing our patient’s clinical data.”

As part of the Trust’s commitment to work with healthcare partners to give patients the right care in the community, Medway Maritime Hospital has been working alongside Medway Council and local GP services since June 2016 to deliver PACE. Through the programme, elderly local residents can continue to live healthy and independent lives with the support from the expanding number community centres opening across Medway. It has also meant that patients that need medical care but do not require emergency care at the hospital can be treated efficiently and with high quality expertise at their nearby centre.

Staff at the Trust were also delighted to become finalists in the ‘Excellence in Quality and Safety’ award with nominee Dr Ghada Ramadan, and the ‘Outstanding Team Achievements for Business Professionals’ award for work demonstrated by the Human Resources and Communications team. A great achievement for all involved.
